Our client is currently seeking a Assistant Director of Social Services
Looking for an Assistant Director of Social Services for their Beverly Location. Candidate must be licensed in the state of Massachusetts. Must have experience in Skilled Nursing Facility.


This job will have the following responsibilities:

• Assist in planning, developing, organizing, implementing, evaluating, and directing the facility's social service programs. Participate in discharge planning, resident assessments, and developing social care plans. Interview residents/families to obtain social history. Refer residents/families to appropriate community agencies when needed. Provide consultation to staff and community agencies on meeting residents' social/emotional needs. Interpret residents' social, psychological, and emotional needs to the care team. Assist in obtaining resources from community agencies to meet residents' needs. Ensure confidentiality of residents' protected health information.
  
  

Qualifications & Requirements:

• The job requires a Bachelor's degree in Social Work, at least 1 year of supervised experience in healthcare, and Massachusetts Licensure
