Salary: Depends on Experience

Description: The Judge Group is currently seeking a Home Health Physical Therapist in North Carolina!


This Home Health Physical Therapist will cover either Stanly, Montgomery or Cabarrus Counties performing SOC, evaluations, reassessments and discharges. This role requires about 50-75 miles of driving a day. Routine visits around 6 patients/day. Sign on bonus offered!



Shift: 8:30a-5p



This job will have the following responsibilities:

• Evaluates, plans, directs, and administers programs of Physical Therapy.


• Plans and conducts Physical Therapy programs to restore function, prevent disability, and help patients enhance physical and functional abilities.
  
  


• Documents evaluations, tests, progress notes, daily treatment notes, attendance records, patient/family education, outcomes, discharge notes, and charges as needed.
  
  


• Communicates patient's evaluation and subsequent progress to physician, referral sources, and other clinical staff, coordinates patient's schedule and treatment programs, and discharge planning.
  
  


• Etc...

Qualifications & Requirements:

• NC license for Physical Therapy required.


• Bachelor's degree in Physical Therapy program required.
  
  


• Possess and maintain Basic Life Support for Healthcare Provider certification from the American Heart Association required.


• Must have home health experience.


• Covid Vax
