Job Title: Physical Therapist - Home Health


Job Summary: We are seeking a dedicated Physical Therapist to join our Home Health team. In this role, you will perform comprehensive evaluations, assessments, and treatment plans for patients in their homes. You will collaborate with other healthcare professionals and oversee the work of Physical Therapy Assistants.


Location: Home Health in Stanly, Montgomery, and Cabarrus Counties. Open to all counties, Physical Therapists will drive approximately 50 to 75 miles per day.


Responsibilities:

• Conduct evaluations and assessments to develop appropriate patient care plans.


• Administer modalities, procedures, and exercise programs based on patient needs.


• Document patient evaluations, progress notes, treatment records, and other necessary documentation.


• Communicate patient progress to physicians, referral sources, and other clinical staff.


• Provide supervision and guidance to Physical Therapy Assistants and Rehab Tech/Aide.


• Adhere to safety protocols and maintain a fast-paced clinical environment.

Requirements:

• Valid North Carolina license for Physical Therapy.


• Bachelor's degree in Physical Therapy program.


• Basic Life Support for Healthcare Provider certification from the American Heart Association.
