Summary:
- The Counselor role oversees all aspects of a patient's treatment. Responsible for the
coordination and continuity of care from admission through discharge and follow-up of the
patients.
- The Counselor role also promotes performance improvement and service delivery by
providing comprehensive psychological therapy directed towards addressing the causes and
effects of the patient's addiction by providing substance abuse counseling for assigned patients
including family and group therapy.
Essential functions:
- Essential functions, including how an individual is to perform them and the
frequency with which the tasks are performed; the tasks must be part of the job function and
truly necessary or required to perform the job.
• Interface with the clinical team in decision-making process involving patients to include
attending all clinical meetings.
• Assess the counseling needs of the patient.
• Assess the patient for substance abuse diagnosis using DSM criteria in determining
appropriate level of care based on ASAM criteria.
• Obtain previous records that are relevant to the current treatment episode.
• Prepare the substance abuse counseling portion of the patient treatment plan.
• Provide substance abuse counseling services specified in the patient treatment plan.
• Develop the patient discharge/continuum of care plan to ensure movement to the
appropriate levels of care.
• Participate fully as a member of a multidisciplinary team for assigned patients.
• Document all counseling and education services, assessments, reassessments, referrals,
and follow-up in the patient's clinical record.
• Maintain patient caseload (including all case management and utilization review
updates, treatment planning duties, and clinical charting as required).

• Adhere to all applicable HIPAA (Health Insurance Portability and Accountability Act) and
42 CFR Part 2 guidelines to ensure confidentiality of patient information.
• Knowledge of legal policies and regulations in the healthcare industry.

• Ability to utilize a wide variety of office technology and tools to collect / enter / process
/ file patient records and documentation.
Bachelors Degree Required
IN (Indiana) Specific Requirements:
• Counselors must be credentialed as any of the following under the Department of
• Mental Health and Addiction (DMHA):
• A Licensed Clinical Social Worker (IC 25-23.6-5) with an addiction credential
• indicated in #8;
• A Licensed Mental Health Counselor (IC 25-23.6-8.5) with an addiction
• credential indicated in #8;
• A Licensed Marriage and Family Therapist (IC 25-23.6-8) with an addiction
• credential indicated in #8;
• A Licensed Clinical Addiction Counselor (IC 25-23.6-10.5);
• A Psychologist (IC 25-33);
• A Physician (IC 25-22.5-2);
• An Advanced Practice Nurse or a Certified Nursing Specialist (IC 25-23-1);
• An individual credentialed in addictions counseling by a nationally recognized
• credentialing body approved by the division as indicated below:
o A Licensed Addiction Counselor (LAC)
o A Certified Alcohol and Drug Addiction Consultant II (CADAC II)
o A Certified Alcohol and Drug Addiction Consultant III (CADAC III)
o A Certified Alcohol and Drug Addiction Consultant IV (CADAC IV)
o A Certified Alcohol and Drug Addiction Consultant V (CADAC V)
o An Addiction Professional in Training (APIT)
o A Certified Addiction Counselor Level I (ICACI)
o A Certified Addiction Counselor Level II (ICACII)
o A National Certified Addiction Counselor, Level I (NCAC I)
o A National Certified Addiction Counselor, Level II (NCAC II)
o A Master Addiction Counselor (MAC)
