Job Title: Nurse Practitioner - Home Health Risk Assessments
Location: Will be working in patient homes within Smith County, TX (Ideally located in Tyler, Lindale, or Flint)
Overview: Our client is a leading healthcare organization dedicated to providing exceptional care to patients in the comfort of their homes. We are currently seeking skilled Nurse Practitioners to join our team.
Job Details:
Part-time: 2-3 days a week (10 - 15 visits), with scheduling based on needs and NP's availability (needs may fluctuate throughout the year)
Ongoing contract position
Compensation: Best rates per assessment, plus mileage reimbursement
Job Description: As a Nurse Practitioner, you will be responsible for conducting Health Risk Assessments (HRAs) through house calls within an assigned area. In this role, you will:
Travel to patients' homes, nursing homes, or other health facilities to perform comprehensive history and physical assessments
Document acute, chronic, and long-term healthcare issues accurately and efficiently using our EMR system
Ensure all appropriate consent forms are signed and dated
Interact frequently with patients, coders, schedulers, and supervising physicians
Educate patients and their families on chronic medical conditions, preventative care, and medication adherence
Complete referrals to case management, nursing, pharmacy, and other services as needed
Participate in clinical meetings and compliance trainings (primarily web-based, with occasional on-site training)
Review patient records for past and present health issues
Collaborate with the supervising physician and other members of the healthcare team
Interpret laboratory data and diagnostic testing when available to assist in diagnosing medical conditions
Perform ADL and Fall Risk evaluations, Depression and Nutritional Health Screenings, BMI evaluations, and medication reconciliations
Perform additional duties as assigned by the supervising physician, such as diabetic retinal exams, bone density scans, etc.
Qualifications:
Family Nurse Practitioner certification required (Adult NP may be considered if no pediatric patients will be seen)
Board certification by the ANCC or AANP
Active/unrestricted NP license within your state of practice (new graduates will be considered)
Ability to be credentialed by Medicare, Medicaid, and other private insurance companies
Proficiency with various computer-based applications (MS Office, logging into programs, navigating different software, typing) and utilizing websites to perform required duties
Strong background or interest in primary care and preventative medicine (preferred)
Access to a personal car with adequate automobile insurance
Highly motivated, self-directed professional with strong organizational skills and the ability to work independently
Home internet connection
Excellent clinical assessment and analytical skills
DEA certification may be required depending on state of practice.
