Schedule
Monday-Friday 8am to 5pm
What Youll Do
Coordinate discharge and aftercare plans with hospitals, crisis units, and community partners.Perform intake assessments and connect individuals to essential mental health services.Develop person-centered plans and provide crisis interventions and safety planning.
What Were Looking For
Experience: 3+ years in community mental health or related fields.Skills: Strong interpersonal, documentation, and time-management abilities.Flexibility: Able to work non-traditional hours and handle high-pressure situations.
