Job Title:Care Management ExtenderDepartment:University Hospital | Social Work and Case Management Administration



Scope of Position


Case Management collaborates with health care team members to develop a plan of care for each assigned patient from admission through discharge. Ensures patient progresses towards desired outcomes by monitoring patient care through assessments and/or evaluations. Identifies and resolves barriers that hinder effective patient care. Actively involved in discharge planning process.





Position Summary


The Care Management Extender will provide programmatic support services for the Care Management department covering in University Hospital, Ross Heart Hospital, Brain and Spine, East Hospital and Dodd Hall. The Care Management Extender will be responsible for coordinating inclusive of throughput/capacity management, discharge planning, and delegated functions at the direction of the Care Manager and/or Social Worker.






Minimum Qualifications



For Hire: Bachelors degree preferred, or combination of education and experience.
