Position Overview:
As an NP, you will play a vital role in conducting Medicare Wellness Visits in partner clinics and occasional home visits, optimizing the health and well-being of patients.


Key Responsibilities:
- Conduct comprehensive wellness visits, including medical history reviews, physical examinations, and health assessments.
- Provide personalized health advice and education to patients.
- Identify and address potential health risks.
- Document patient encounters accurately and timely using our EMR system (eClinicals).
- Collaborate with a multidisciplinary team to deliver coordinated care.


Qualifications:
- Current California NP License and Board certification
- Active DEA license.
- Minimum of 2 years of experience in primary care, hospice, or urgent care/outpatient settings.
- Fluency in Spanish preferred.
- Proficiency in eClinicals or similar EMR systems.
- Knowledge of HCC coding.
