Job Summary Great opportunity to join an experienced Case Management team. Role focuses on transition planning and Utilization Review.
RN Case Managers are assigned geographically to inpatient units and paired with a Care Team to facilitate through put.
This position is for unit-based case manager with focus on assessment and development of transition plan .
Utilization Review is embedded in this role - team uses InterQual and provide training in use of this product.
Looking for good critical thinking skills and willingness to learn in a fast-paced environment.

Qualifications
Required:
High degree of skill in written/oral communications and personal interactions. Demonstrates agility in a continuously changing fast paced work environment. Demonstrates computer competency. Ability to complete daily job responsibilities within normal hours by moving rapidly through the hospital and physician offices. Must be able to perform the essential job functions, either unaided or with the assistance of a reasonable accommodation to be determined by the organization on a case-by-case basis. Culturally competent.
Three or more years of clinical experience in a health care delivery system.
Completion of an accredited Nursing program.
Current Nursing License in the State of MA is required.
Preferred:
Case Management, Home Care Discharge Planning experience
