Job Summary
This is a hybrid role working 10am - 6:30pm M-F and rotating every other weekend, 7am - 5pm.
Key Responsibilities
• Referral information is reviewed to ensure compliance with established agency guidelines and compliance with local, state, and federal regulations including homecare, and/or lifeline.
• Referral process is coordinated to ensure a smooth transition of patients to MGB Home Care in accordance with the RSC procedures and established timeframes.
• Patient assessment information is obtained from a variety of sources, Electronic, Fax, Phone and data is transmitted in appropriate system (4Next).
• Referral requests for services are processed in 4Next ensuring all information is complete and accurate according to RSC policies.
• Clinical and non-clinical data is updated and corrected in the 4Next system as appropriate.
• Collaborative relationships are developed and maintained with internal staff to ensure customer satisfaction.
• Referral source relationships are nurtured to ensure and maintain customer satisfaction.
• Complex admissions are supported using internal clinical and supervisory resources.
• Pertinent information from MGB Home Care is communicated to the current medical provider to meet patient and provider needs.
• Account/territory referral and admissions targets are achieved.
• Educational information and updates are shared with the referral sources to ensure an understanding of the benefits of MGB Home Care
• Referral requests are reviewed for clinical appropriateness to determine acceptance for services according to MGB Home Care standards.
Physicians' orders for home care are reconciled with initial referral and reviewed for clinical significance as needed.
• Referral requests for services are processed ensuring all information is complete and accurate according to Intake policies as needed.
• Clinical and non-clinical data is updated and corrected in the 4 Next, as appropriate, and communicated to receivers of referral information
• All referrals with next day SOC, generated by RSC liaison, are completed and finalized by end of day.
• All referrals with next day SOC, generated by RSC liaison, have physician orders by end of day or SOC is delayed by a day
• Communication is practiced in an effective and efficient manner according to Referral Service Center guidelines of excellence in customer service
• Referral source relationships are nurtured to ensure a high level of customer satisfaction
Education/Degree Requirements
• Associate degree or equivalent clinical education
Licensure, Certifications or Registration
Required: LPN or other clinician with current MA license required (RN, PT, OT, PTA)
Experience
• LPN or other clinician with current MA license required (RN, PT, OT, PTA)
• Minimum 2 years homecare, acute hospital, extended care, case management or managed care environment required.
• Computer literate with ability to learn new software applications.
• Strong written, verbal and electronic communication skills.
• Highly developed interpersonal and communications skills with ability to interact with a diverse population.
• Strong organizational skills with aptitude for obtaining detailed information accurately.
• Understanding of insurances preferred.

Referral request information is reviewed for clinical appropriateness to determine acceptance for home health services and to ensure certified home health guidelines are met. Referrals are accepted for services and fully completed in the MGB Referral Platform. (4Next) Review and reconcile physicians' orders and update medical data. Communicate pertinent information to involved parties according to RSC and MGB Home Care guidelines. Primary contact will be with physician practices, acute care facilities, skilled nursing and extended care facilities for both current and potential patients. Ensure that physician orders and all Medicare required documentation is received and communicated to appropriate staff. Maintain professional and collaborative external relations with referral sources. Uphold the MGB Home Care Organizational Values of Innovation, Collaboration, Accountability, Respect, and Excellence.
