This is a dual position working as a Social Worker and Case Manager.

Job Summary
Assess patients and families from a social work and case management perspective; complete necessary clinical documentation; provide education to patients/families regarding insurance coverage and community resources; make referrals to appropriate community agencies/resources; participate in interdisciplinary team process/collaborate with the interdisciplinary team to formulate treatment plans to maximize emotional, social and physical well-being.
Qualifications
Key Responsibilities:
• Clinical social work assessments and MDS entries are completed within the timeframe established and include each assessment element established in the Clinical Social Work Scope of Practice guidelines.
• Case management/social work plan is developed collaboratively with the interdisciplinary team, patient and family to maximize health care responses, quality and cost effective outcomes, incorporating patient and family goals, expectations, and preferences into the development of the interdisciplinary plan of care
• Clinical reviews are submitted to third party payors/insurers in a timely manner
• Culturally competent care is displayed to identify the patient's cultural needs and incorporate them in the development of the discharge plan.
• Patient education and support is provided to ensure a smooth transition from acute setting, through the SRN stay and transition to discharge using established standards.
