Position Summary:
The LPN Care Navigator plays a crucial role in the healthcare system, providing support and guidance to patients throughout their healthcare journey and is a part of the multidisciplinary Ambulatory Care Management Team. They identify and advocate for patients' needs and preferences within the healthsystem, ensuring they receive appropriate care and services.
The role of the LPN Care Navigator is to outreach high risk, complex medical patients to identify patient barriers to care. The LPN Care Navigator will assist in helping patient's navigate the healthsystem by linking patients with appropriate resources and members of the multidisciplinary care management team (RN, Social Worker, Pharmacist). The LPN Care Navigator serves as a liaison between patients and their primary care provider, ensuring seamless coordination of care to overcome barriers, increase adherence to care, and improve patient outcomes.
Key Responsibilities:
Interacts regularly with patients and/or patient caregivers to identify barriers preventing the adherence to care plans.
Provides direct patient outreach to identified high-risk, complex patients to identify barriers to care
Facilitates appropriate referrals to address patient care needs
Assists with patient enrollment in Chronic Disease Management programs
Documents and maintains accurate patient records within care connect
Interact with members of the multidisciplinary care management team to discuss improvement of process, outcomes and adjusting to meet patient needs
Collaboration with GRIPA and RRH Care managers
Other duties as assigned
EDUCATION:
BS: Nursing (Required)
PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
LICENSES/CERTIFICATIONS:
RN - Registered Nurse - New York State Education Department (NYSED)
BLS - Basic Life Support - American Heart Association (AHA)
PAY RANGE: $23.75 - $29.75
