Job Summary:

The Health Federation seeks a highly qualified and energetic individual with excellent organizational skills and experience with patient navigation to support implementation of projects focused on cancer screening and navigation to care. The ideal candidate will have experience working directly in a health care setting, experience in project/program management, outstanding interpersonal skills, and experience working with individuals living in communities experiencing poverty, discrimination, and lack of access to services.

Responsibilities/Duties:


  • Project Management:

    • Develop and monitor work plans and assure that deliverables are met

    • Plan and implement outreach and communication activities, such as mailings, phone calls, and texting campaigns.

    • Coordinate with team members internally and at partner organizations to assure that deliverables are met or to adjust work plans as needed.

    • Schedule and coordinate partner meetings to discuss updates, progress and challenges.

    • Organize necessary reports and develop presentation decks to share with partners and stakeholders

    • Identify and implement training and development opportunities for participating organizations

  • Patient Navigation:

    • Using technology tools, work with health care partners to identify patients in need of navigation to additional diagnostic testing and/or additional care.

    • Provide holistic navigation assistance to support quality of life goals for patients

    • Develop a network of key points of contact at health centers, health systems, community based organizations, insurers, and other relevant entities to support patient success.

    • Support growth of cancer care/resources database including support groups, fitness, nutrition, mental health etc.; develop relationships for referrals and warm hand-off.

    • Document all interactions with patients, collaborate regularly with supervisor to address challenging cases and identify solutions

    • Integrate best practices into workflow to support optimal outcomes

    • Assess and address patient needs to enable attendance at appointments and engagement in care

    • Consult with clinical team members to assure accurate information is provided to patients.

  • Personnel Management

    • Provide training and oversight for volunteers, consultants, vendors and other identified personnel as needed to achieve program goals.

    • Manage agreements, hours and work assignments

  • Technology Support:

    • Use Azara DRVS/CareConnect, Redcap, Smartsheet and Google for Business, among other platforms, to meet project goals.

    • Assist collaborators in utilizing platforms effectively to manage data and documentation

    • Provide input into the design of tools to support and improve on successfully reaching project goals.

Education:

Bachelor’s degree in public health, health administration or related field and/or relevant clinical degree or licensure required.  In exceptional cases, relevant work experience may substitute for a degree.

Skills/Experience


  • A minimum of three years of work experience focused on patient navigation and/or project/program coordination is highly desirable.

  • Compassionate and empathetic

  • Highly organized and able to manage complex timetables, budgets, and competing tasks and priorities.

  • Experience in a patient or client focused role, working with communities impacted by poverty, discrimination, and lack of access to services.

  • Excellent written and verbal communications skills

  • Interest in and ability to learn software programs quickly. Some background with electronic health records, databases, and/or population health tools highly desirable.

  • Fluency in Spanish or another non-English language spoken in Philadelphia highly desirable.

Work Environment: Standard office setting with extended periods at work station and periodic use of office equipment.

Position Type and Work Schedule:  Full time position, 37.5 hours per week. HFP’s hybrid work policy requires employees to be in office or in the field 3 days per week.

Travel:  Occasional local travel to meetings and trainings.

Physical Demands: Position requires sitting at a computer and desk and mobility around the office. Occasionally transports supplies and equipment weighing up to 20 pounds.
