Mountain View Post Acute is seeking a dedicated and experienced Resident Care Manager to lead and support our team in delivering exceptional care. This role requires a current LPN or RN license and a passion for making a difference in the lives of our residents.

Your Responsibilities:

• Oversee and coordinate resident care plans to ensure quality and compliance


• Provide leadership and guidance to nursing staff


• Monitor resident outcomes and adjust care plans as necessary


• Ensure proper documentation and communication of care


• Collaborate with interdisciplinary teams to enhance resident well-being

Requirements:

• Must hold a current and active LPN or RN license
  


• Previous experience in a leadership or care management role preferred


• Strong organizational and communication skills
