Job Description:

Compensation: $25.54/hour

Due to critical service need of the communities we serve, we are offering a $1500 sign-on bonus.; additional $500 for Qualified Mental Health professionals.* We are eager to continue to provide quality and compassionate care in our service areas and need you to make this happen!

The Special Services Coordinator is responsible for a variety of specialty and support services that assist Adapt of Missouri clients. Provides direct and indirect support to outreach programs utilizing assertive outreach and engagement approaches. Manages/assists with special client programming including but not limited to Nursing Home Support Program, Intake and Outreach Programs. Assists teams as needed to provide direct client services. Sees clients in a variety of community settings and neighborhoods.

ESSENTIAL FUNCTIONS - JOB SPECIFIC

* Coordinates with Nursing Home Staff
o Maintains communication with DMH staff regarding impending referrals into the program
o Attends meetings as needed with DMH staff
o Meets with and develops rapport with possible referrals prior to their discharge to Adapt
o Assesses client's level of risk, strengths, and weaknesses
o Interviews clients, reviews records and confers with other professionals to evaluate client's preparedness for step down
o Participates in routine care coordination meetings with Adapt staff
o Develops and maintains positive relationships with skilled nursing facilities where clients are or might be housed
o Actively engages clients to develop and provide interactive and engaging interventions
* Collaborates with members of the outreach team to assist with engaging and outreaching identified clients referred to Adapt through ERE, DM3700 or other referrals sources
* Assists with client services as needed including Open Access and Outreach
* Completes documentation for billable and non-billable services provided
* Performs job duties with an efficient use of time based on client's needs as well as meeting efficiency requirements based on budget parameters
* Demonstrates the knowledge of legal issues and civil rights relevant to work setting
* Conducts activities in a culturally competent manner
* Keeps supervisor informed by meeting with supervisor for individual supervision as scheduled
* Updates supervisor on possible issues or other pertinent staff or business issues
* Is open to constructive criticism and makes apparent efforts in areas of self-improvement
* Attends in-service trainings given and participates in meetings held by the company
* Attends outside trainings to as part of own professional growth
* Demonstrates efficiency, flexibility and attention to detail
* Assumes any other responsibilities as directed



• Bachelor’s degree in human services field required
• Master’s degree in counseling, psychology, social work, or related mental health field preferred

QUALIFICATION(S): WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS

• Four years of experience in mental health field and/or experience working with adults with chronic mental illness if Master’s degree isn’t obtained
• Must have an automobile, a valid Missouri Class E driver’s license and agency established minimum automobile coverage with required coverage limits

QUALIFICATION(S): LICENSURE/CERTIFICATION

Must obtain and maintain certification in CPR, First Aid and CPI and Medication Level I Aide
