As a Clinical Care Coordinator, you will...
Facilitate clinical care coordination services for clients in need of enhanced care coordination.
Ensure that clients receive timely and efficient support services though a multi-disciplinary care plan including referrals to both internal and external resources.
Perform data collection, reporting, and surveying.
Oversee the coordination of care across programs.
Participate in Continuous Quality Improvement activities.
Participate in huddles.
Schedule and assure patient access to care.

To qualify for the position, you will need...
An associates degree
At least one (1) year of experience working in a health or behavioral healthcare setting or in a similar capacity.
Ability to work 40 hours per week within agency hours of operation, which are 8am-8pm Mon-Thurs, 8am-4pm Friday with Saturday hours, when applicable.
