JOB SUMMARY:



The role of the Tobacco and Nicotine Cessation Facilitator (TNCF) is to reduce and eliminate tobacco and nicotine use of patients and community members seeking support to quit. The TNCF engages people interested in attending a smoking cessation group to create supportive networks and share smoking cessation tips to quit smoking. The primary responsibility is to plan and conduct weekly cessation groups, provide outreach, and follow up to facilitate long-term quit status. This role serves as a subject matter expert within the cancer center and health care system related to smoking cessation strategies. In addition, The TNCF provides resources and information for individuals to access local and state quitlines, nicotine replacement therapy options, and other possible pharmaceutical and cessation tools. The TNCF is responsible for data collection of group attendance, quit rates, and other relevant metrics.



Primary Responsibilities:

• Plans and facilitates smoking cessation groups and one-to-one interventions for tobacco and nicotine cessation.


• Assures the physical environment of the group is conducive to positive group dynamics and is comfortable for all attendees.


• Develop and organize content and hand-outs for use during cessation groups.


• Uses skills to assess individual motivations and readiness to quit.


• Provides evidence-based intervention, support, and resources to interested individuals to optimize a successful quit.


• Works in partnership with individuals to develop a quit plan, identifying and accessing appropriate nicotine replacement therapy and pharmacological resources, and other strategies for achieving a successful quit.


• Works collaboratively with healthcare providers and team members to receive referrals and provide support around smoking cessation to individuals seeking support.


• Conducts outreach calls to attendees to provide support and encourage group attendance.


• Collects data and prepares reports around program attendance and outcomes, including smoking status post interventions.


• Documents interventions provided via electronic medical record.


• Perform all other duties as assigned.

Qualifications

Education & Experience - Required:



• Health Education Specialist, Registered Nurse, licensed Social Worker (Bachelor or Master), Clinical Psychological Counselor, or related health care field with certification in or eligibility to obtain certification from a nationally recognized tobacco cessation certification program (ie American Lung Association, Council for Tobacco Treatment Training Program, American Heart Association etc,)


• Relevant experience may include leading health-related groups, knowledge of the impact of smoking, tobacco, and vaping on health and well-being, experience with one-to-one counseling on health issues, understanding of cessation models and motivational interviewing techniques, and other behavioral and psycho-pharmacological interventions for cessation. Former smokers or vapers (minimum of 1 year quit) are encouraged to apply.



Knowledge, Skills, & Abilities:



• Strong communication skills to promote and support smoking and/or vaping cessation.


• Understanding of motivational interviewing techniques and other evidence-based behavioral change strategies.


• Understanding and ability to manage group dynamics.


• Ability to create a positive, supportive, and safe group environment.


• Strong Interpersonal and organizational skills.


• Knowledge of Epic or other electronic medical record systems


• Ability to use basic office computer programs and databases such as Microsoft Office

Compensation:



Pay Range: $26.14/hr-$29.53/hr-$36.59/hr
