Description: Our client is currently seeking a Care Manager RN in St. Cloud, Florida!


As a Care Manager RN, you'll be at the heart of managing transitions of care, ensuring seamless coordination and support for patients. Join our dynamic team of 17 members, including 3 dedicated providers, and thrive in a collaborative and supportive environment. Your expertise in triaging, scheduling, and coordinating with hospice and VITAS services will be invaluable in providing top-notch care.

Job Type: Contract to Hire



Location: St. Cloud, Florida 34769



Hours: 8:00 AM - 5:00 PM (Monday - Friday)



Salary: Competitive and Dependent on Experience



Job Responsibilities:

• 
  


• Provide comprehensive patient care.


• Manage hospital updates and discharges.


• Address transition of care needs and schedule accordingly.


• Coordinate with hospice and VITAS services.


• Track and follow up on hospitalizations.


• Triage patient needs effectively.


• Ensure seamless communication between patients and healthcare providers.


• Maintain accurate and timely documentation.

Required Qualifications:

• Registered Nurse License


• 
  


• Case management experience


• Strong healthcare background

Preferred Qualifications:

• Bilingual
