Facility: Wellstar MCG Health



The Social Worker assesses the social needs of the assigned patients/families coordinating a plan of care that meets the unique needs of the patient and his/her family. Collaborates and participates in the disciplinary planning process and coordinates pre/post-hospital care, and effective use of community resources to integrate care, improve quality of care, and decrease cost and utilization.


Position Requirements:


Master's Degree in Social Work and 2 years or more of experience in Healthcare or hospital setting. Excellent assessment skills; strong organizational skills. Ability to work independently and team setting. Current American Heart Association or American Red Cross Basic Life Support (BLS).
