Join our dedicated healthcare team as a Diabetes Case Manager, where you will play a crucial role in improving the lives of individuals managing diabetes. In this impactful position, you will collaborate with healthcare professionals to develop personalized care plans, provide education and support to patients, and ensure seamless coordination of care. Job Responsibilities
- Assess and evaluate patients’ medical history and current health status to develop individualized care plans for diabetes management.
- Provide education and counseling to patients and their families on diabetes management, including blood sugar monitoring, medication adherence, and lifestyle modifications.
- Collaborate with multidisciplinary healthcare teams, including physicians, dietitians, and educators, to coordinate comprehensive care for diabetic patients.
- Monitor and track patients’ progress, adjusting care plans as necessary to achieve optimal health outcomes.
- Facilitate access to resources such as diabetes education programs, support groups, and community services.
- Develop and implement strategies to improve patient self-management skills and compliance with treatment protocols.
- Conduct regular follow-up appointments and communicate effectively with patients to address concerns and provide ongoing support.
- Document patient interactions, care plans, and progress notes accurately in the electronic medical record system.
- Participate in quality improvement initiatives to enhance diabetes care delivery and patient satisfaction.
- Stay updated with the latest research, guidelines, and best practices in diabetes management to provide evidence-based care.
- Advocate for patients’ needs and work to remove barriers to effective diabetes management within the healthcare system.
Job Requirements
Licensure: Any state (but AZ preferred)
Certifications: Certified Diabetes Educator (CDE) preferred, (AHA) BLS
Schedule: Mon - Fri, 8a - 5p
Volume: Approximately 15 per day either face-to-face or telephonically
Ages Seen: All ages
Experience: Working knowledge of diabetes treatment tasks and program management
