Position Title:Care Manager - Adult Targeted Care Management

Job Summary: The Case Manager facilitates coordination, communication and collaboration on behalf of the client with a severe and persistent mental illness diagnosis. They work together in order to achieve goals and maximize outcomes, connecting clients with relevant resources, information, education, and communicating/collaborating with professional and natural supports to aid clients in reaching their preferred goals.
Essential Functions:
Analyze client and guardian (if applicable) situations in order to provide comprehensive, person-centered support to clients 18 and over with an SMI diagnosis who reside in the 7-county region and have active Medicaid coverage.
Perform face-to-face contacts and needed consultations with other service providers/professionals and natural supports who are involved with the overall treatment plan of the client. A minimum of 4 contacts/consultations are required by state contract, with two being face-to-face and two being collateral.
Through analysis of available client data, client/guardian contacts, and team member consultations, the Case Manager will facilitate team in determining if all identified treatment needs are being addressed, identify unmet needs, and provide intervention and advocacy on behalf of the client/guardian in the home, community, or client’s work/school setting.
Coordinates and assigns needed services that are identified in the Client’s Service Plan, which is compiled by the client and the Case Manager and noted in the EMR. The individual plan is developed, modified and reviewed as needed, but at least quarterly.
The Case Manager will advocate for the client/family interest concerning services, funding sources, treatment/program alternatives, community resource activities and life choices, etc., promoting the protection of client/guardian rights and ensuring due process in the event of violations/restriction of rights. The Case Manager will educate, promote and encourage clients in self-advocacy whenever possible.
Case Managers will complete required Core Competency and TCM Specialized Training within 6 months of hire, as required by state contract.
Case Managers will complete required client paperwork in Avatar within most current stated timeframes and will complete all contact notes within 48 hours of service provision.

Qualifications:
Education and Experience:
1. Bachelors Degree in a behavioral science from an accredited institution. Behavioral sciences include Psychology, Social Work, Sociology, Human Services, Special Education, Family Studies, Counseling, and Nursing.
2. One (1) year paid experience in case management or working with the chronically mentally ill population after Bachelor's Degree in a behavioral science is completed. A Masters Degree in a behavioral science can substitute for the 1 year of experience.
3. Valid driver’s license and reliable transportation.
4. Good oral and written communication skills.
5. Physical ability to do light lifting.
6. State Case Management certification within six (6) months of employment.
7. All Case Managers are required to complete and pass the Safe Crisis Management 2-day class prior to independent functioning. The SCM training must be re-certified annually.
8. No more than 25 cases are to be assigned to each Case Manager based on state contract.
