Job Summary:
The medication utilization pharmacist is responsible for planning and implementing formulary management and medication use initiatives in conjunction with other members of the Center for Medication Utilization team. The pharmacist also participates in monitoring and improving adherence to medication management strategies. The medication utilization pharmacist supports System P&T and associated subcommittee activities across the Froedtert enterprise. Other responsibilities include medication shortage management, core reference oversight, and development and maintenance of intranet web pages. The individual is engaged with physician, pharmacist, nursing, and multidisciplinary education regarding new medications and initiatives. The pharmacist serves as an expert in drug information specialty practice, including practical application of literature evaluation. The medication utilization pharmacist precepts pharmacy students and residents. The individual supports the organization's efforts around medication safety, medication alerts and recalls, REMS, and other safety-related activities. The individual works with other Center for Medication Utilization team members to advance the Froedtert Health mission and strategic initiatives. The pharmacist supports compliance with The Joint Commission and Center for Medicare/Medicaid standards for pharmaceutical services. The medication utilization pharmacist reports to the Director of Medication Use and Finance. Other duties as assigned.
EXPERIENCE DESCRIPTION:
PGY1 pharmacy or managed care residency or minimum 3 years of experience in a hospital is required with demonstration of extension projects in drug use policy or formulary management.
PGY2 Drug Information or PGY2 Medication Safety residency training, preferred. Candidates with PGY2 training in a specialty clinical practice area with demonstration of extension projects in drug use policy or formulary management will also be considered.
EDUCATION DESCRIPTION:
Pharmacy degree received from an ACPE certified school of pharmacy is required.
Graduate from an ASHP-accredited residency program from an academic medical center (AMC) or AMC-affiliate hospital is preferred.
SPECIAL SKILLS DESCRIPTION:
Demonstrated ability to work in a team environment, prioritize multiple longitudinal projects, and time management is required. Strong written and verbal communication to facilitate change and build rapport with the multidisciplinary health care team. Proficient in Microsoft® software products, including manipulation of Excel data and understanding of technology principles
Expertise in advanced literature searching strategies, interpretation of various study design, and a concrete understanding of statistics is required to complete day-to-day responsibilities. Demonstrates professional motivation through involvement in local and national organizations with active contributions back to the profession through peer review, poster presentations, or article publication. Knowledge of medical writing principles and styles of writing (eg, AMA or NLM). Examples of work may be requested.
LICENSURE DESCRIPTION:
Wisconsin Pharmacist Licensure is required and must be obtained within 6 months of hire.
