Job Title: Case Manager RN
Job Type: Contract to Hire
Location: Medford, Oregon 97501
Hours: 8:00 AM - 5:00 PM (Monday - Friday)
Rate: Competitive and Dependent on Experience
Primary Responsibilities:
Ensure accurate and complete documentation.
Communicate and collaborate with the interdisciplinary care team.
Assist the provider/team with various care coordination activities.
Assist the provider/team with benefit determination associated with Medicare Part A, Part B, and Part D benefits.
Collaborate with the primary advanced practice clinician (APC) and facility to identify and respond to patient changes in condition.
Assist the provider/team with therapy coordination.
Participate in facility-based meetings.
Schedule and participate in family conferences, team meetings, and team case presentations.
Coordinate training, tracking, and compliance with quality measures.
Assist in creating and conducting in-services for selected audiences.
Assist providers in ensuring complete and accurate documentation and coding, medication reconciliation, nursing rounds, assessment, and patient documentation.
May write verbal orders from the provider in the chart in compliance with state RN practice laws and individual nursing facility practices.
Assist in managing members through acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions.
Assist in managing members during short-term sub-acute rehabilitation episodes.
Required Qualifications:
Active and unrestricted license in the state of residence, or ability to obtain by start date.
Associate degree or higher in Nursing (RN).
Certified in Basic Life Support.
Ability to complete physical requirements, including lifting a 30-pound bag and performing physical assessments.
Access to reliable transportation & ability to travel between facilities.
Proficient computer skills, including the ability to document medical information with written and electronic medical records.
Effective communication skills and time management.
Ability to work with diverse care teams in a variety of settings.
Proof of full COVID-19 vaccination prior to employment.
Candidates are required to pass a drug test before beginning employment.
Preferred Qualifications:
1+ year of clinical experience in practice (long-term care setting preferred).
Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
Understanding of geriatrics and chronic illness.
Travel Details:
Location: Medford, OR- including Jackson and Josephine Counties
Timing: During shift hours
Facilities: Traveling between 2-3 facilities
