Description: Our client is currently seeking a Nurse Practitioner/Physician Assistant

Duties and Responsibilities
- Conduct Health Risk Assessments (HRA) on insured plan patients in their homes or plan designated areas
- Identify all active and chronic disease conditions as well as determine all physical, mental, and social needs present at the time of the visit
- Obtain vital signs, measure BMI, review pharmacological therapy and conduct a physical examination
- Conduct necessary screenings to capture HEDIS measures and close gaps in care
- Educate and counsel patient and family on any conditions identified during assessment
Candidate Requirements
- Must have clear, active Nurse Practitioner state license and board certifications
- 1 year experience preferred
- Providers enrolled in PECOS preferred
- HCC experience/ knowledge preferred
- Ability to complete and clear a background and credentialing screen
- Must have valid state driver license and automobile insurance
- Candidate will be required to travel within the county or counties selected
- Strong computer skills: including but not limited to Word, Excel, EMRs and Web based applications
- Must be able to complete all HRAs and corrections (if needed) in a timely manner
- Must be able to complete a minimum of 30 visits monthly
