The Judge Group is looking forNurse Practitionersto performin-home health assessments on Medicare Advantage Members in Lincoln County, OR with one of the largest healthcare providers.
Pay- HIGHLY competitive, paid per assessment.
Contract- ongoing (no end date)
Schedule- Flexible schedules. Ability to work any day of the week from 7AM - 8PM. (can be made around another current position). At least 30 assessments per month are required.
All training and equipment are provided by the client.
1099 position.
Mileage reimbursement from assessment to assessment.
Travel reimbursement for hotel lodging (when applicable).
A mix of in-home and virtual assessments.
Overview:
The provider will review and observe a member's current health, medical history, medication adherence, social environment, and other risks in about a 1-hour non-invasive in-home visit (non-prescribing, non-treating). This provides unmatched insight into a member's overall health and well-being that can be difficult to capture during routine office visits.
Responsibilities
Conduct Health Risk Assessments (HRA) on insured plan patients in their homes or plan designated areas.
Identify all active and chronic disease conditions and determine all physical, mental, and social needs present at the visit.
Obtain vital signs, measure BMI, review pharmacological therapy and conduct a physical examination.
Conduct necessary screenings to capture HEDIS measures and close gaps in care.
Educate and counsel patients and family on any conditions identified during assessment.
Qualifications:
Have a Minimum of 1 year of experience as a Nurse Practitioner.
Master's Degree required OR commensurate experience and satisfactory completion of NP licensure.
Current unencumbered RN and NP licensure in state of OR.
Board certified by the AANP, ANCC in an approved specialty.

Acute Care
Adult Health
Family Health
Have a current BLS, ACLS or CPR certification.
Have strong computer skills and familiarity with Employee health/medical record software.
