Position Summary:
The MDS Coordinator is responsible for the day-to-day coordination and oversight of all aspects of the RAI and Care Planning process in accordance with Federal and State
regulations.
Daily duties:
Scheduling of MDS assessments
Completion of MDSs
Completion of RAPs
Complete/update Care plans
Utilize the Daily Care Plan Update sheet and work collaboratively with the nursing team to ensure implementation.
Attend daily meetings as assigned
Conduct Resident Care Conferences per facility schedule
Communicate with interdisciplinary team members
Track admissions and discharges
Attend morning meeting/clinical report as assigned
Computer Application/Data Entry
Weekly duties:
Transmit MDS's to State
Attend weekly resident "At Risk" meetings
Work collaboratively with nursing team to properly utilize and implement the MDS Alert Sheet.
Monthly duties:
Complete monthly calendar for MDS completion and Resident Care Conference 2. Attend and participate in the Quality Assessment and Assurance meeting
Attend and participate in the QI/QM investigation meeting.
Attend all monthly staff meetings
Quarterly Duties:
Attend facility communication meetings
Attend Regional MDS Coordinator meetings
Other duties as directed by the Director of Nursing/Administrator
Recommended Minimum Position Qualifications:
Education:
Graduate of an approved LPN/RN program.
RN preferred.
Professional Experience:
MDS Coordinator experience preferred.
Certification/Licensure:
Actively licensed as an LPN/RN in assigned state(s) of employment.
