What You'll Do:



  • Jump into Action: Evaluate new patients promptly after referral from their physician to assess their needs and get them started on the road to recovery. If there's ever a delay, you'll document the reasons clearly in the patient record.


  • Craft Personalized Treatment Plans: Collaborate with the referring physician to develop effective, individualized treatment plans and gain approval for services. You'll follow through with treatments that help your patients reach their full potential.


  • Guide and Lead: Assist in training Restorative Aides and supervise Physical Therapy Assistants, ensuring the highest standards of care are always met. You'll be their go-to expert, following the state practice act and your expert judgment.


  • Collaborate and Communicate: Stay connected with your supervisor and other team members about patient progress, needs, and adjustments. Participate in patient care conferences, utilization reviews, and rehabilitation meetings, making sure that everyone is on the same page.


  • Teach and Empower: Engage with families and staff, leading caregiver training and providing vital instructions on maintaining therapy programs after discharge. Your guidance will be key in helping patients regain their independence.


  • Innovate and Problem-Solve: Stay on top of necessary durable medical equipment, such as walkers, braces, and grab bars, ensuring that patients have everything they need to stay mobile and safe.


  • Maintain Documentation Excellence: Record all treatment notes, evaluations, and progress updates in compliance with PT Board State Practice Act and payer requirements, ensuring your work is always transparent and accountable.


  • Continuous Learning: Be a part of our in-service training programs for the entire facility and expand your skills in an ongoing learning environment.


Supervisory Responsibility:
  • Lead and supervise Physical Therapy Assistants, aides, and students, helping them grow and learn from your expertise.

What You Bring:


  • Education/Experience: A Bachelor's in Physical Therapy (Master's or Doctorate is a plus!) and proficiency with software like Casamba and Point Click Care.


  • Skills & Abilities:

    • Solid understanding of technical procedures, policies, and medical documentation

    • Strong communication skills, both written and verbal, to engage with all levels of staff, patients, and families

    • Problem-solving abilities to tackle the day-to-day challenges of rehabilitation

    • The ability to apply math concepts like ratios, percentages, and proportions to real-world scenarios




  • Licensing: Must be licensed as a Physical Therapist in your state with active continuing education requirements, always maintaining your license in good standing.


Physical Requirements:


  • Stay Active: You'll be on your feet a lot - standing, walking, and occasionally sitting to engage with patients. You'll frequently push, pull, and lift, making sure equipment is in working order and patients are receiving the best care.


  • Strength & Stamina: You may need to move heavy objects (up to 100 pounds), but don't worry - it's all part of the job!
