Summary: Oversees the development and implementation of the community consortium plan aimed at reducing disparities in perinatal health outcomes in Smith County. Develops, implements, and conducts outreach, education, and recruitment of eligible families for program services. They are from, and a representative of the community served.

Essential Functions:
Oversee the development and implementation of the Community Consortium plan. Organize and coordinate Consortium meetings. Coordinate community-based education (topics discovered by Consortium members) to facilitate the overall goal of reducing disparities in perinatal health outcomes within Smith County. Assists with the coordination and promotion of Healthy Start sponsored events and meetings; including Consortium, support groups, and community events. Builds collaborative relationships with the community, health care providers, community leaders, service agencies, policy makers, funding sources and target audiences. Develop cross-sector partnerships within the project area. Participate with other HS/Healthy Start Initiative-Enhanced (HSE) projects to develop and implement a plan for the community to focus on one social determinant of health (SDOH). Determine eligibility of families for program services and state benefits. Complete all required documentation and supporting documents for intake and state benefit application prior to completion of shift. Submit state benefit applications and supporting documents through YourTexasBenefits website. Enter participant intake information into Salesforce and create a file. Maintain list of applications' status and update Salesforce database. Provide safe, secure, and supervised transportation for the service population assigned to this position for transport. Provide referrals to families based on needs identified while conducting outreach efforts. Inspect company vehicles and fill-out check list prior to using the vehicle. Facilitate meetings and/or training for program families
Measurable Deliverables:
1. Meet all pre-determined deadlines required by program director, federal, and state partners.
2. Meet all federal/state regulatory guidelines and standards that are applicable to this position.
3. Document all daily outreach efforts, application assistance, health education classes and consortium activities completely, accurately and in a timely manner and in compliance with agency policies and procedures.
4. Schedule promotion efforts consistently throughout the fiscal year.
5. Schedule community consortium events consistently throughout the fiscal year.
6. Refrain from ever using a handheld cell phone while operating a vehicle.
7. Obtain and enroll in continuing education/training required for recertification(s) monthly.
8. Provide consistent and effective communication with program supervisor for any concerns or issues relating to the community consortium and/or community outreach efforts.
9. Implement BCFS safety protocols, at all times.
10. Maintain BCFS HHS professional and ethical standards of conduct outlined in BCFS employee handbook including demonstrating respect for agency staff, families, and community members and complying with required dress code, at all times.
Credentials, Skills and Abilities
1. Maintain current licenses including any certificates required for the position including, but not limited to: Driver's License, First Aid CPR AED, Application Assistance Navigator, Community Health Worker, Mental Health First Aid, and Child Passenger Safety Technician.
2. Pass a pre-employment drug screen and random drug screens throughout employment.
3. Provide proof of work eligibility status upon request.
4. Pass pre-employment and biennial criminal background checks.
5. Drive a vehicle and comply with safety procedures in accordance with program requirements. 6. Demonstrate the ability to:
a. Respond sensitively and competently to the service population's cultural and socio-economic characteristics.
b. Work collaboratively with other staff members, service providers, and professionals
c. Work in a fast-paced environment and maintain emotional control, and professional composure, at all times.
d. Communicate effectively in writing and verbally in English (Spanish highly preferred)
e. Organize and prioritize responsibilities and duties efficiently.
f. Maintain computer literacy required to meet the responsibilities of the position.
g. Maintain confidentiality in all areas of the service population and program operations
