Location: Rolla, Missouri
Compensation: $34.73/hourly-$46.45/hourly Dep on Licensure
The Hospital Access Liaison (HAL) Supervisor oversees the HAL team to provide clinical direction and ensure quality care is being provided to clients who have been recently discharged from the hospital.
This is a Full-time, benefit eligible position. The schedule is Monday - Friday 8:00am-5:00pm.
ESSENTIAL FUNCTIONS - JOB SPECIFIC:
* Provide supervision of Hospital Access Liaisons including at least one hour face-to-face individual supervision per week, daily availability for consultation, and ongoing opportunity for skill development.
* Ensures that appropriate documentation and recording occurs within individual case records according to policy
* Has a working knowledge of CHN program eligibility requirements.
* Participate in Hospital Diversion management meetings
* Collaborates with hospitals state-wide to maintain and build new relationships
* Coordinates care internally with all CHN programs (crisis, CPRC, SUD, outpatient, etc.)
* Has the ability to travel for internal and external meetings, supervisions, and trainings.
* Conducts regular team meeting/huddles
* Ensures that clients are outreached within 24 hours of hospital discharge.
* Develop new processes as needed to improve quality of care
* Ensure clients are being connected to appropriate services within 30 days of hospital discharge
* Maintains data tracking system for accurate reporting
* Consistent focus on client quality of care post-hospitalization, including 7- and 30-day hospital discharge appointments.
* Conducts assessments for potential consumers
* Other duties as assigned
Master's degree in social work, psychology or related field required
* At least one (1) year experience working in behavioral health required
* Working knowledge of DSM-V required
* A valid MO driver's license with agency-established minimum automobile coverage required
QUALIFICATION(S): LICENSURE/CERTIFICATION: PLPC, LPC, LMSW, LCSW, PLP or LP required
