CLINICAL COMPETENCE:
Optimal patient care outcomes from drug therapy are ensured through clinical pharmacy services. Such services are age appropriate, for neonates, pediatric patients, adolescents, adults and geriatric patients, performed when needed through direct contact with the patient, and in direct association with or in support of health care providers.

Therapeutic Drug monitoring: Drug therapy monitoring includes an age appropriate assessment of:

Drug-drug, drug-food, drug-laboratory, or drug-disease interactions.
Clinical and pharmacokinetic laboratory data to evaluate efficacy of drug therapy and anticipate side effects, toxicity, or adverse effects.
Physical signs and symptoms relevant to the patients; drug therapy.
Provides oral and written consultations on request with other health care practitioners regarding drug therapy selection and management.
Administers medical-staff approved collaborative drug therapy protocols
Provides age and educationally appropriate patient education and counseling on request regarding drug therapy and drug-related disease prevention
Develops patient-specific, age appropriate, drug therapy management plans and therapy endpoints
Documents medication-related care in patient’s medical record.
Monitors, detects, documents, reports and manages adverse drug reactions.
Educates health care practitioners regarding drug therapy.
Participates in drug use evaluations, quality assurance and continuous quality improvement programs.
Participates in formulary control and maintenance.
Provides accurate, comprehensive, patient specific drug information to pharmacists and other health care practitioners as appropriate.
Participates in drug and drug-related research.
WORK FLOW/PRODUCTIVITY:
Accurate, efficient quality medication-related care is ensured by utilization of positive management techniques in supervision coordination and assignment of appropriate personnel.
Responsible for the filling of all patient medication orders during assigned shift.
Responsible for completion of all assigned work procedures during the shift without interference to quality of work performed.
Delegates other responsibilities in order to keep patient care the number one priority.
Regularly assists other pharmacists and pharmacy technicians as time allows and as requested.
Responds to changes in work load as necessary. Uses time in productive manner-assists technicians & other pharmacists, restocks, cleans, orders, organizes, special projects, etc.

Responsible for dispensing and oversight of dispensing functions.

Maintains pharmacy patient profiles.
Compounds and prepare ordered pharmaceuticals as appropriate.
IV admixture: including TPNs and chemotherapy
Filling outpatient prescriptions as required.
Working knowledge of pharmacy equipment.
PROFESSIONAL DEVELOPMENT:
Pharmacy knowledge and skills are updated, shared and applied continuously to improve quality of pharmaceutical care.
Attends pharmacy related Continuing Education in a regular manner as required by state law.
Inservices appropriate hospital staff on knowledge, procedures, and application of content from educational programs attended.
Plans and implements new policy and procedures based on advances in pharmacy knowledge and technology.

Patient, personnel, department and facility safety and security is ensured by knowledge and application of established policies and procedures, safety, security and emergency plans.
Maintains accurate inventory and records on narcotics as required by law.
Maintains certification and demonstrates knowledge of required safety, security and emergency programs required by department or hospital.
Maintains clean, safe and orderly environment.
Consistently demonstrates a high degree of accuracy with minimal risk management incidents unrelated to system problems
Education:
Graduate of accredited School of Pharmacy.
Skills:
Keyboard skills and ability to navigate electronic systems applicable to job functions.
