Job Description:

The DAC Peer Support Specialists responsible for providing re-entering individuals with Serious Mental Illness (SMI) and/or Severe and Persistent Mental Illness (SPMI), who are on the Department of Adult Correction (DAC) prioritization list, with education and support throughout the transition process.

***This is a provisional appointment meaning it is expected to have an end date within the next 12 to 18 months.***

This is a full-time remote opportunity. There is no expectation of being in the office routinely, however, the selected candidate must reside in North Carolina and be e available to travel to the Alliance office as needed for meetings. This position will also require travel throughout the Wake, Harnett, Johnson, Cumberland and surrounding areas to support Alliance members in various settings.

 Responsibilities & Duties

Provide Care Team Support


  • Support members transitioning from Inpatient settings to the appropriate lower or lateral level of care

  • Provide subject matter expertise, within scope, regarding member’s physical and/or behavioral health to support the development and delivery of a whole person approach to Care Management 

  • Work collaboratively with other Alliance staff, behavioral health providers, primary care physicians, specialty care providers and other community partners and stakeholders to support members in their home communities

Support DAC Care Coordinator with Core Re-Entry Transitional Care Coordinator Functions


  • Meeting with clients regularly to offer one-on-one support and counseling

  • Required travel to meet with members regularly to offer one-on-one support and counseling

  • Developing recovery plans, coping methods, and rehabilitation strategies with the client

  • Assisting members with developing short-term goals, coping methods and rehabilitation strategies with members

  • Arranging medical appointments, therapy sessions, support groups, and other resources to aid the client

  • Reporting any unusual behavior, conflicts, or recovery deviations to the relevant professionals

  • Reporting any challenges, conflicts, or barriers to DAC Care Coordinator

  • Liaising with family members and medical professionals regularly

  • Working with potential members family and medical professionals to address members needs and goals

  • Coordinating the client's logistics such as medical appointments and therapy sessions, among others

  • Addressing a member’s barriers including, not limited to, medical appointments, treatments, and other important appointments for services

Minimum Requirements

Education & Experience

Required:

Must have a High School Diploma or equivalent and have NC Certified Peer Support Specialist Program Certification (NC CPSS). At least 1 year background and expertise working with people with SMI/SPMI and their families or guardian. Lived experience with mental health conditions, substance abuse or trauma is required. Candidate must reside in NC and must be willing to travel.

NC Certified Peer Support Specialist Program certification is required.

Preferred:

Experience providing peer supports to individuals in and out of the Justice System. 

Knowledge, Skills, & Abilities


  • Computer proficiency in Microsoft Excel, Word and Outlook.

  • Ability to utilize computer equipment and web-based software to conduct work.

  • Ability to interact with various office staff as needed to support necessary workflows.

  • Ability to interact with healthcare professionals, patients, their families and other supports.

  • Ability to communicate effectively to individuals and groups through spoken, written and electronic media.

  • Ability to attend to detail, effectively prioritize and execute tasks in a timely manner.

  • Ability to work independently without a high degree of supervision.

Salary Range

$25.00-$33.75/Hourly 

Exact compensation will be determined based on the candidate's education, experience, external market data and consideration of internal equity.
