This position will serve as the regional Traditional Health Worker (THW) resource for providers and community-based organizations to advance the integration of and access to THWs. The THW Liaison is responsible for building and expanding the THW network. They will act as the hub for THWs in the CCO service area, providing THW-specific technical assistance to provider partners and prospects to advance TWH integration, access, and best practices. The THW Liaison will work closely with providers, community stakeholders and the Provider Network Programs team to develop the annual THW Implementation and Utilization Plan to increase regional availability of THW services. This position will be responsible for collaborating across departments to collect data and develop reports to meet regulatory requirements.
Essential Responsibilities:
Serve as the regional THW resource for providers and community-based organizations to advance the integration and utilization of THWs.
Provide technical assistance and education for providers on THW scope of practice, benefits, supervision and availability of THW services. Includes the development of materials, webinars, and other educational documents. Promote education through PacificSource mediums and external presentations.
Advance the integration of THWs into the delivery of contracted services in collaboration with internal stakeholders and based on best practices. Outreach to current and future employers of THWs to develop THW contract prospects. Develop and maintain THW prospect lists.
Develop THW baseline utilization measures and performance indicators over time in collaboration with Analytics and the Provider Programs team. Use measures to identify integration barriers and develop strategic solutions.
Develop the annual THW Integration and Utilization Plan. Ensure strategies increase recruitment and retention of THWs in the provider network, are aligned with the provider workforce development program, and are compliant with OHA regulations and the OHA Traditional Health Worker Commission's best practices under the Office of Equity and Inclusion.
Implement PacificSource's THW Integration and Utilization Plan. Prepare annual THW Implementation and Utilization Plan Report to meet OHA requirements. Contribute to other required reports including the Delivery Service Network (DSN) report.
Develop effective solutions and manage schedule, timelines, activities, and milestones to ensure thorough development and timely implementation of projects. Routinely monitor and report on progress.
Participate in the development of the member engagement plan and THW payment methodologies.
Represent PacificSource THW interests with provider and community-based THW organizations to build collaborative partnerships that advance integration efforts in alignment with PacificSource provider workforce development strategies.
Collaborate with the OHA THW Commission and Office of Equity and Inclusion to implement best practices and meet regulatory requirements.
Actively participate in various internal and external committees in order to disseminate information, represent company philosophy, and advance THW best practices.
Supporting Responsibilities:

Follow the PacificSource privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.
Perform other duties as assigned.
Work Experience: Three years of experience in healthcare, public health or community-based organization. Prefer experience as a THW or in health care coordination services. Experience preferred in strategic relationship development, training, and/or facilitating groups.
Education, Certificates, Licenses: Bachelor's degree required or equivalent combination of education and experience required. THW training and certification strongly preferred.
Knowledge: Knowledge and understanding of social determinants of health and health equity. Commitment to diversity and inclusion. Experience with program implementation. Relationship building and community collaboration. Experience giving presentations. Organizational skills with solid experience in using computers and various software applications. Ability to work independently. Bi-lingual strongly preferred.

Environment: Work inside in a general office setting with ergonomically configured equipment. Travel is required approximately 50% of the time. Rare overnight travel.
