Key Responsibilities:

• Develop and oversee individualized social service care plans, supporting residents' emotional and social well-being


• Act as an advocate for residents, ensuring their needs and concerns are addressed and respected


• Coordinate with healthcare providers, family members, and community resources to support comprehensive care


• Ensure compliance with state and federal regulations and maintain accurate documentation


• Lead and mentor the social services team, promoting a collaborative and compassionate care environment

Qualifications:

• 
  Education: Bachelor's degree in Social Work or a related field


• 
  Experience: Prior experience as a Social Services Director or similar role in a healthcare or post-acute setting


• 
  Licensure: Licensed Social Worker in Alaska (or eligibility to obtain licensure)
