Key Responsibilities:
Develop and implement individualized treatment plans for patients recovering from illness, surgery, or injury
Provide direct physical therapy services, focusing on mobility, strength, and functional independence
Collaborate with an interdisciplinary team to ensure the highest quality of patient care
Document patient progress and adjust treatment plans as needed
Educate patients and families on therapy techniques to continue at home
Qualifications:
Licensed Physical Therapist (PT) in the state of California
Experience in a skilled nursing or post-acute care setting preferred
Strong communication and interpersonal skills
Passion for improving patients' quality of life
