Purpose of Position:

The primary purpose of this position is to develop, as a member of a multidisciplinary team, a psychosocial assessment and treatment plan for each patient with a specific focus on successful community reintegration and incorporating prior community functioning, forensic encumbrances, inpatient and outpatient treatment needs, family involvement and patient preferences and goals.

Key Responsibilities:

Clinical Services

  • Conducts diagnostic interviews, observations and contacts the community treatment providers, forensic coordinator, probation officer, entitlement providers and family members and/or significant others.
  • Performs evaluation of admission materials and obtains data essential for treatment planning and documents same in a full psychosocial admission assessment according to current department guidelines.
  • Collaborates with treatment team members to determine whether further diagnostic studies or information are needed.
  • Participates in developing individual Master Treatment Plans with treatment team members and take the lead in development of a transition and discharge plan.
  • Coordinates the completion of all applications for discharge including housing, financial, work, and psychosocial outpatient programs.
  • Ensures that legal encumbrances are adequate addressed and insures timely follow up with probation and parole officers.
  • Attends court hearings at Circuit, District, and Family courts as needed on all islands.
  • Provides individual and group psychotherapy, counseling, supportive therapy, training, or other modalities of treatment in compliance with the patient's Master Treatment Plan.
  • Establishes and maintains a therapeutic relationship with every patient assigned to the inter-disciplinary team.
  • Coordinates inter-agency staff reports for patients considered appropriate for discharge.
  • Prepares and maintains statistical forms and reports related to patient admission, changes of legal status and discharge and documents same in patient’s medical record.
  • Plans and coordinates appropriate discharge and aftercare for each patient on the assigned caseload and documents the plan throughout hospital episode and on the Discharge Summary.
  • Ensures that proper education and counseling to family, caretakers, and/or other persons significant to the patient's treatment team are provided, and documents same in-patient record.
  • Completes all documentation required by the hospital/department according to current guidelines within required time frames.
  • Provides in-patient care management documentation to include treatment goals and interventions, patient treatment plans, patient responses, and related medical data.
  • Maintains 30-day documentation of post discharge care received by the patient.
  • Provides documentation, gathers data pertinent to establishing a case for court hearing, identifying key witnesses of the dangerousness displayed and determining the patient's mental condition, stability, and post hospital plans.
  • Assists the Attorney General's Office and/or criminal court staff in preparing an individual case for hearing as appropriate.
  • Testifies in court case hearings on or off Hawaii State Hospital (HSH) property regarding clarification of issues of appropriate restrictive facilities or as an eyewitness to the element of dangerousness.

Education and Experience:

Applicants for Health, General Welfare or Corrections Welfare positions must meet the requirements specified below in addition to A or B:

In accordance with Act 238, SLH 2005, applicants must possess a bachelor’s or master’s degree in social work from a program of study accredited by the counsel on Social Work Education, or a doctoral degree in social work from a college or university accredited by the Western Association of Schools and Colleges or a comparable accreditation body.

The education background must also demonstrate the ability to write clear and comprehensive reports and other documents; read and interpret complex written material; and solve complex problems logically and systematically.

  • In addition to meeting the requirements specified for level III, applicants must have had a minimum of one additional year of professional social work experience equivalent to level III. Such experience must have demonstrated ability to perform the most difficult and complex assignments.
