SCOPE OF ROLE
Reporting to the Program Manager, Behavioral Health Community Liaison, the Liaison, Recovery Outreach Team will work as part of a multi-disciplinary team including law enforcement, corrections and behavioral health. The Liaison will assist the Lake County Narcotics Agency (LCNA) to provide services intended to prevent and reduce impact of opiate use, outreach, linkage, and education to identified individuals who have experience overdose, with the goal of helping them to accept necessary treatment. The Liaison will work with LCNA to provide education related to drug awareness and the dangers of opioid abuse, to students based on developmental level as well as partner with the LCNA and the Lake County Sheriff's Office to coordinate and implement mental health/drug rehabilitation services.
HOW YOU'LL SUCCEED
Coordinate mental health/drug rehabilitation services for drug dependent individuals who recently experienced and overdose, low-level offenders and drug dependent individuals encountered by LCNA agents during drug investigations and school students within Lake County through home visits.
Coordinate with local emergency departments and community behavioral health agencies to identify individuals currently linked with community behavioral health agencies, or in need of intervention and referral.
Provide referral and support family members, drug education events and outreach activities.
Foster the relationship between law enforcement and mental health professionals.
Provide advocacy and outreach on behalf of individuals who are unable to advocate for themselves due to symptoms of mental illness and addiction.
Identify, develop, and access support resources for patients to increase their success in community integration and community inclusion.
Partner closely with identified individuals and family members in critical or crisis situations.
Represent Signature Health in a professional manner at all times; includes maintenance of appropriate professional appearance as outlined in the dress code policy, and demonstration of respectful interactions with patients and team members.
Requirements
KNOWLEDGE & EXPERIENCE
Minimum High School Diploma or equivalent required.
2+ years' experience in a related field, such as behavioral health, corrections, law enforcement or addictions strongly preferred.
Valid unencumbered Ohio driver's license and proof of driver's insurance required.
American Heart Association (AHA) Basic Life Support (BLS) certification required.
Demonstrated knowledge of addiction, ability to reflect non-judgmental, unconditional acceptance of individuals suffering from this condition required.
WORKING CONDITIONS
Work is normally performed in a typical interior/office/clinical work environment or clients home/ community setting in conjunction with local law enforcement, Lake County Narcotics agency and community behavioral health agencies.
Hours of operation depend on needs of the program. It is expected that this will include several evenings per week, and will require flexibility.
Requires periods of sitting, standing, telephone, and computer work.
Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.
Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.
Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.
Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.
Possible exposure to blood borne pathogens while performing job duties.
Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.
Sufficient dexterity to operate a PC and other office equipment.
