TITLE: Community Health and Wellness Area Agent NKY (Boone, Kenton, Campbell Counties)
DEPARTMENT: Cooperative Extension Program
REPORTS TO: Northern KY Area Director
CLASSIFICATION: Staff-12 months
EFFECTIVE DATE: TBD
STANDARD WORK HOURS: 37.5 hours per week
EMPLOYMENT STATUS: Full-Time 
EMPLOYMENT CLASSIFICATION: Exempt 
 
JOB SUMMARY: The Community Health and Wellness (CHW) Agent will promote wellness by assisting communities in the adoption of healthy behaviors. CHW agents will by provide information, tools, and encouragement to empower individuals and communities to improve overall health. CHW agent(s) will help bridge cultural knowledge and health literacy differences and improve communications involving community members and health agency. CHW agents will work with individuals, families, community members, and healthcare providers to identify strategies and solutions to increase healthy behaviors among underserved and underrepresented communities.The Community Health and Wellness agent will be responsible for teaching in person and virtual programs to families, individuals, and small groups. The CHW agent will sit on advisory boards, attend programs and events in targeted communities and schools as directed to promote and recruit individuals for programming. The CHW agent will assist in programmatic efforts and work in collaboration in with the KY Health Mobile Unit, State Specialist for Nutrition and Health Equity and other nutrition research and extension professionals. 
 
ESSENTIAL JOB FUNCTIONS:

  • Provide culturally appropriate health education workshops, group discussions and demonstrations on topics related to chronic disease prevention, physical activity, and nutrition

  • Help community members get appropriate screenings and referrals for health and social services

  • Implement programs and advocate for people who may have limited access to health resources and social services

  • Identify and share appropriate information and resources to help individuals, families, groups, clients, and organizations meet their needs.

  • Evaluating community health needs

  • Initiate and sustain trusting relationships with individuals, families, and social networks

  • Gathering and interpreting county and state health needs assessments

  • Research evidence-based interventions that best suit a communities need

  • Advocating for policies and resources that improve community health while building community capacity to address health issues

  • Create connections between vulnerable populations and healthcare providers

  • Address social determinants of health

  • Attend all training meetings and conferences.

  • Work collaboratively with other community agencies and resources.

  • Other related duties as required

    KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:
     
  • Experience working with youth, adults, and/or families who come from diverse socio-economic backgrounds.

  • Convey the importance of the following public health concepts and approaches:
  • Maternal and Child Health

  • Prevention of injuries/falls risk,

  • Infectious Diseases

  • Chronic Diseases

  • Mental and Behavioral Health

  • Racial and Health Equity

  • Trauma-informed/responsive approaches

  • Ability to learn and present basic nutrition information and implement educational events.

  • Knowledge of local community organizations and agencies.

  • Oral and written communication skills.

  • Strong interpersonal and relationship-building with the ability to work effectively with a wide range of constituencies in a diverse community.

          SUPERVISORY RESPONSIBILITY This position does not have supervisory responsibility. 
     
               
    OTHER DUTIES: Position will perform other related duties as required
     
    QUALIFICATIONS: 
  • Bachelor’s degree in Public Health, Nutrition, and or related fields of study. Master’s degree preferred

  • Possess a valid Kentucky driver's license and insured automobile for business mileage reimbursement.

    WORKING CONDITIONS AND PHYSICAL EFFORT:  
  • Must be able to lift moderate sized boxes

  • Must be able to travel (25%)

  • Must be able to work evenings and weekends (50%)
