Job Summary: The Care Partner is a support care provider who functions under the direction of the Registered Nurse. The Care Partner primary focus will be to promote a positive patient experience by partnering with the nurse, physician, healthcare team and other healthcare providers to provide excellence in patient experience; and assist with daily living activities and comfort measures. The Care Partner assists other health care providers with safe, age appropriate and culturally aware care by performing basic patient care functions; i.e. ADL's, ambulation, and vital signs. Approved advanced technical training skills may be required i.e. phlebotomy, simple treatments, dressing changes, oral suctioning, and discontinuing Foley catheters. Performing additional advanced skills may be required to support clinical care and will fluctuate depending on the needs of the unit. Performing clerical functions such as: retrieval of data, maintaining unit supplies, and transporting supplies/patient medications will be required. The Care Partner must be able to work in a fast-paced environment which requires multi-tasking and effective communication with the Registered Nurse and ancillary team. Must be able to perform under stressful conditions with a compassionate, respectful and service mindset. Must be able to take direction from a variety of care providers. Must be organized with excellent communication skills. Core Responsibilities and Essential Functions: Excellence in Customer Service by partnering with health care team, patient and family - Protects rights and dignity of resident at all times. - Maintains confidentiality. - Participates in resident rounding per standards. - Answers telephone and call lights promptly and notifies appropriate individual of needs for - follow- up. - Resolves resident concerns and immediately informs the Charge Nurse/manager of - complaints. - Proactively offers assistance and comfort measures to residents and families. - Present self in positive and professional manner and show sensitivity to concerns and needs - of others. Resident Centered Care - Provide resident care according to the written Plan of Care. - Provide basic resident care needs (ex. hygiene, nutrition, elimination, physical comfort - measures) as assigned. - Assist Charge Nurse/Manager and other members of health care team with care delivery as - assigned. - Provide meals and nourishment to the resident according to dietary standards and the plan of - care. - Take vital signs and document, report verbally to the Charge Nurse any unusual findings. - Document all patient care delivered in a timely manner as required by policy. Provide for a safe and organized environment - Perform basic housekeeping duties in the immediate area of the patients room and bath. - Cleans and sanitizes personal direct care items (bedpan, urinal, etc) per policy/standards. - Ensure that safety hazards related to fall risk are removed or controlled i.e. cords, oxygen - tubing, clutter etc. - Ensure that Oxygen signage is displayed, and tanks are stored correctly. - Ensure that bedrails, bed alarms and personal alarms are utilized correctly and encourage - family and visitors to do so. Ensure that the resident has all the required supplies needed for care - Stock the necessary medical supplies and skin care products in the resident care area. - Stock the required linens and incontinent supplies in the residents room. Provide emotional support for the resident and family. - Provide time to listen and establish rapport. - Affirm and recognize the familys role as the decision makers and caregivers - Recognize the value of the life of the resident. - Provide a peaceful presence. - Observe any religious or cultural restrictions or practices of the patient or family.
