The University of Iowa Healthcare, University Campus is seeking a Discharge Planning Assistant. The Discharge Planning Assistant is an active member of the care coordination team, working collaboratively with all areas within the Care Coordination Division. The Discharge Planning Assistant is responsible for performing administrative and operational functions needed in organizing the Care Coordination Division. This position will provide direct administrative support to the discharge planning process as well as utilization management functions.
The Discharge Planning Assistant will work in a fast-paced, high-volume environment multitasking between telecommunication and web-based applications to deliver a high level of customer service in a timely manner. This position will work with in a variety of clinical areas including the Inpatient units and Emergency Department and other internal and external customers.
Provide direct support to the all departments within the care coordination division.
Verify the presence of a Clinical Power of Attorney.
Potentially distribute regulatory notices and completes all required documentation.
Assist with securing patient resources as requested.
Assist with post-acute referral and monitors status, reaching out for screening determinations as necessary.
Support the care coordination discharge functions by submitting payor authorization and required medical record documentation to insurance providers and third-party vendors.
Assist in obtaining information for VNA agencies, DME vendors, rehabilitation facilities, etc.
Potential collaborate on requests for facilitation of the discharge plan with the Nurse Navigator or Social Worker to ensure timely progression of inpatient to discharge at the next level of care.
Demonstrate thorough knowledge of various computer/information systems to perform assigned duties, including but not limited to Indicia, EMR, Excel, and Outlook
Collect/input data or information in appropriate databases as indicated by the Care Coordination leadership.
Independently identify and communicate any problems or issues that affect departmental goals and outcomes.
Perform basic administrative tasks related to the job as required by the Care Coordination Division to maintain accurate records and to ensure worker accountability/productivity.
Maintain a highly acceptable level of professional conduct and respect for medical staff, coworkers, and hospital staff to foster a desirable image for the institution.
Maintain compliance with all hospital/departmental policies/procedures assigned by the department manager, including work hours, scheduling, and other criteria for the expected daily operations of the department.
Maintain strict confidentiality in dealing with all patient-related activities and other sensitive physician and/or hospital issues by strictly adhering to hospital confidentiality of information policies.
Acknowledge budgetary constraints in department operations and strives to perform duties cost-effectively and efficiently.
Demonstrates ability to prioritize multiple work assignments to accomplish the assigned workload.
Performs other duties as may be assigned to ensure that departmental objectives are fulfilled.

Required Qualifications
Bachelor's Degree or an equivalent combination of education and experience is required.
A minimum of 6 months experience in health care.
Excellent written, verbal, and interpersonal communication skills.
Must be proficient in computer software applications.
Desired Qualifications
Knowledge and understanding of medical terminology.
Knowledge of Post-Acute community resources.
Strong organizational and prioritizing skills
Experience in working with EPIC
