Description: Job Title: Advanced Practice Clinician (Nurse Practitioner/Physician Assistant)
Job Overview: One of our largest clients is looking for an Advanced Practice Clinician (APC) to join their team. We are looking for a provider to join their integrated care delivery program that coordinates clinical care for members in a facility setting. As an APC, you'll provide care to our highest-risk health plan members as part of an interdisciplinary team.

Work Schedule: Monday to Friday, 8am to 5pm
Primary Responsibilities:
Conduct comprehensive assessments.
Manage acute and chronic medical and behavioral conditions effectively.
Ensure accurate and complete documentation, including ICD 10 conditions.
Collaborate and communicate with the interdisciplinary care team.
Conduct advanced illness and advanced care planning conversations.
Provide counseling and education to patients and caregivers.
Required Qualifications:
Active and unrestricted license in the state of residence, or ability to obtain by start date.
Education:
NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
PA: Graduate of an accredited Physician Assistant degree program.
Active Nurse Practitioner or Physician Assistant certification through a national board.
Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
Access to reliable transportation
Ability to gain a collaborative practice agreement.
Preferred Qualifications:
1+ year of clinical experience in practice (long-term care setting preferred).
Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs.
Understanding of Geriatrics and Chronic Illness.
Understanding of Advanced Illness and end of life discussions.
Proficient computer skills including the ability to document medical information with written and electronic medical records.
