Job Overview
The Scheduling Social Work position is responsible for the administrative oversight and supervision of direct reports at assigned PACE site that will include master’s level social workers, as well as other PACE team members as needed. Promotes effective collaboration and care planning amongst interdisciplinary team members. Responsibilities include, but are not limited to: coordination of routine assessments, care planning, and other related tasks within the scope of practice; assisting with family conferences, community referrals, auditing team member documentation, onboarding and orientation of new hires as well as routine competency and work performance evaluations. Utilize knowledge of state and federal regulatory requirements as they relate to social work department tasks, in order to provide effective oversight and ensure compliance. Collaboration with other site leaders to monitor site safety, overall staffing changes, and program budgets.
Minimum Requirements
1. Master’s Degree in Social Work (MSW) from an accredited university is required.
2. Minimum of 3 years of experience required.
3. Minimum of 2 years of experience in a supervisory position required.
4. Minimum of 2 years of experience in case management/social advocacy/care planning with geriatric population preferred.
5. BLS certificate is required.
6. Bilingual: Spanish/English, Chinese/English or another second language is strongly preferred.
Compensation
$87,466.87 - $109,333.59 annuallyCompensation Disclaimer

The range is subject to change.
