Job Title:Patient Care Resource Manager, James Cancer Hospital - OutpatientDepartment:James | Outpatient Patient Care Resource ManagerJob DescriptionScope of Position/Position Summary: The Patient Care Resource Manager-Ambulatory is a nurse case manager that functions within the multidisciplinary team as an advocate, educator, and resource for patients. The PCRM is: (1) an advocate by ensuring access to appropriate care and services, promoting self-care management and prevention; (2) a navigator, identifying and removing barriers to care and coordinating patient care services by utilizing the nursing process to help patients move through the healthcare continuum; (3) a collaborator, working within the multidisciplinary team to ensure a comprehensive plan of care in accordance with nationally recognized evidence-based standards; (4) an educator, instructing patients and their caregivers in understanding and managing their diagnoses, treatment, symptoms and clinical trial options; and (5) an expert, knowledgeable in care management principles and the utilization of system and community resources tailored to meet individual patient needs.Required QualificationsBachelors degree required, Masters preferred. RN with 3-5 years clinical experience applicable to the patient population, home care, hospice, community agency, and/or payer relations required. Prior oncology nursing or case management experience preferred.
