JOB RESPONSIBILITIES
Coordination and Preparation - Reports educational progress to patient's school. Collaborates with intradisciplinary team on preparation for transition back to school. Consults with patient and family regarding plans for re-entry. Facilitates the re-entry process following discharge. Participates in patient care conferences.
Interaction - Meets new patients and gathers pertinent information through observation and interview. Assesses educational needs in classroom and at bedside. Makes appropriate recommendations based on accurate assessments. Makes internal and external referrals as indicated.
Assessment - Assesses educational level and cognitive status of patients. Identifies physical readiness and cognitive readiness for participation in school program.
Goal Setting - Formulates appropriate educational goals. Develops curriculum based on individual patient educational goals. Implements educational programming based on educational assessment to maximize growth and development that is age/grade level appropriate.
JOB QUALIFICATIOINS
Bachelor's degree in a related field
0+ years of work experience in a related job discipline
Current teacher certification in the State of Ohio
This position will have 8 weeks off in the summer.
