Major/Essential Functions
Work performed as part of a total health care team in providing patient referrals coordination and navigation including:
Tracking and documentation of patient referrals to promote team awareness and ensure patient safety
Assemble patient demographic, insurance/payer, and health data
Insurance/payer prior authorization
Provide patient and family communication, navigation, and advocacy through the referral process
Establish and maintain communication and relationships with identified service providers and community resources
Update patient records with demographics, insurance, and information on specialist consults, hospitalizations, ER visits, and community organizations related to their health.
