The LPN is a licensed practical nurse who participates in the plan of care to provide competent, compassionate and individualized nursing care to patients and their families - as developed by a registered nurse, nurse practitioner, or physician. Their role is crucial in facilitating clinic flow during clinic sessions by taking vital signs, rooming patients, administering vaccines, performing phlebotomy, assisting with procedures, providing wound care, PICC care and other skilled tasks. The LPN verifies and updates patient information in EPIC, supports prescription refills and performs other duties to support patient care and the clinical teams. As a member of the care team, the LPN will foster therapeutic long-term relationships with their patients and contribute their insights to the care team to improve the patients plan of care.

Qualifications

Proficiency in wound care, venipuncture, and injection administration.

EXPERIENCE:
Previous experience in Ambulatory or Primary Care preferred
