As a Physical Therapist, you will assess, plan, organize, and participate in rehabilitative programs aimed at improving mobility, relieving pain, and preventing deformities due to disease or injury. Services are provided in a cost-effective, quality-focused, and customer-oriented manner, with strict adherence to regulatory standards.
Core Responsibilities:
Patient Assessment
Collaborate with physicians to develop and evaluate treatment programs and care plans.
Create individualized plans of care with input from patients and their families.
Develop realistic discharge plans that align with patient and family goals.
Select appropriate activities to meet patient goals.
Revise treatment goals based on ongoing patient and family feedback.
Provide education and training to patients and families.
Offer discharge instructions and connect patients with community resources as needed.
Documentation
Document evaluations and care plans per departmental guidelines.
Record each treatment session according to protocols.
Complete all required documentation, including family education and discharge summaries.

Required Qualifications:
Education:
Graduate from an accredited physical therapy program (Bachelor's, Master's, or Doctorate in Physical Therapy).
Licenses and Certifications:
Valid Physical Therapist license required.
Basic Life Support (BLS) certification required.
Experience:
Minimum of 2 years of acute or inpatient rehabilitation experience as a Physical Therapist.
Skills:
