Description:
Reporting to the Manager of Social Work
Social Worker will provide assessment, counseling, referral and discharge planning services to all age groups of patients treated at Holy Cross; provides supportive services to patients and their families.
Responsibilities:
Social Workers are focused on discharge and initial inpatient admission and appropriate community-based services.
Reviewed clinical assessments and determining appropriate levels of care, determining if additional clinical services are needed for infants to elderly patient population.
Effectively handled member crisis calls by identifying risk, ensuring safety, and coordinating timely access to care.
What you will need:
State of Maryland LSWA, LGSW, LCSW or LCSW-C licensure (or license eligible).
Masters Degree in Social Work.
One year of social work experience in an acute-care setting preferred. Demonstrated ability to work with all age groups of patients, with an emphasis on adult and geriatric patients. Able to communicate effectively.
Pay rate: $33.73 - $50.59
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
