Job Description:


  • Responsible for reviewing and assessing the quality of pharmacotherapy for patients in assigned coverage areas. Responsible for the optimization of drug therapy by identification, resolution, and prevention of drug related problems and improving therapeutic outcomes.

  • Also responsible for processing medication orders entered in provider-order-entry (and written) by the prescribers, for promptly and accurately providing medications for administration to patients, and for supplying pertinent, timely, and accurate information to health care workers and patients for the safe and effective use of medications.

  • Work independently, without direct supervision, within the limits of established pharmacy practice and within the scope of departmental policies and procedures, as a member of the patient care team. Represent the Pharmacy on key programs, including serving as a liaison, serving on committees as appropriate, recommending policies and procedures, and reviewing drug and supply requirements for that program.

  • Oversees the preparation, compounding, and dispensing of medications to patients in accordance with the laws and regulations of the Board of Registration in Pharmacy, Department of Public Health, Food and Drug Administration, and other regulatory agencies.

  • Provides drug information and pharmacotherapeutic consultations to physicians, nurses, and other healthcare professionals regarding appropriate drug therapy. Provides expert advice on drug interactions, side effects, incompatibilities, allergies and drug administration.

  • Monitors drug therapy to prevent contraindications, duplicate therapy, drug interactions and allergic reactions Recommends appropriate medication doses and adjustments as indicated by patient conditions. Identifies, documents, resolves, and works to prevent drug therapy related problems.

  • Attends and participates in medical and multi-disciplinary rounds on a daily basis.

  • Detects and assists in documenting all clinical interventions, adverse drug events, and drug therapy profile reviews.

  • Provides training/in-service education to Pharmacy staff.

  • Actively participates in the development and performance of drug use educational materials and other drug information tools and monitoring initiatives. Actively participates in keeping the Drug Administration Guideline updated.

  • Participates in cost avoidance and cost savings activities, by working on current or implementing new drug utilization projects designed to provide effective treatment while reducing cost.

  • Provides training/in-service education to Nursing and/or Medical staff.

  • Contributes annually to the Pharmacy Department newsletter.

  • Assists the Clinical Pharmacy Manager with clinical clerkship students and pharmacy externs as outlined by the Massachusetts Board of Registration in Pharmacy and the local colleges of pharmacy.

  • Ensures that proper drug security and control is performed throughout the day.

  • Completed assigned medication inventory assignments.

  • Maintains necessary record keeping.

  • Performs extra duties upon request of Pharmacy Director and/or Assistant Director as assigned.






Qualifications


  • Must have an advanced degree or a Doctorate of Pharmacy from an accredited college of pharmacy, or be currently enrolled in an advanced degree program and have demonstrated advanced clinical skills. Pharmacists with equivalent years of advanced clinical pharmacy practice experience will be considered.

  • Must be licensed to practice in the Commonwealth of Massachusetts.

  • Staff adheres to all C.A.R.E. Standards.

  • Must have excellent oral and interpersonal communication skills, to communicate effectively and work collaboratively with members of other healthcare disciplines, patients, and pharmacy staff.

  • Must understand the basic operations of the Pharmacy Department, including processing prescriber medication orders, the operations of the Omnicell cabinets and software, MediTech, the repackaging (pre-packing) machine, and any other technology that the Pharmacy employs in its operations.

  • Prioritizes and organizes work while under stress and with time constraints. Must be detail oriented, and possess critical thinking and decision making skills.

  • Must maintain sufficient knowledge of pharmacotherapy for the safe and effective distribution of medications.

  • Possess enhanced knowledge of the principles of growth and development over the life span of the neonate, adolescent, adult, and geriatric patient. In addition, the individual must possess the ability to interpret patient data relative to age specific needs and provide care as described in the department's policies/procedures.

  • Must possess leadership abilities in Clinical Pharmacy Practice and be considered one of the clinical pharmacy experts in the department.
