Job Summary: The RN Case Manager plays a crucial role in enhancing the health outcomes of our members. Through the case management process, you will assist members who are facing the challenges of illness and injury.
Job Responsibilities:
Evaluate individual member needs and develop personalized care plans to address their specific health concerns.
Collaborate with healthcare providers, insurers, and other stakeholders to ensure seamless delivery of care.
Track member progress and adjust care plans as needed to optimize outcomes.
Communicate clearly and effectively with members, healthcare providers, and internal teams.
Educate members about their health conditions and advocate for their needs.
Work independently and effectively communicate with internal and external customers.
Assess members' health status, identify treatment gaps or barriers, and create personalized case management plans that involve all relevant stakeholders.
Qualifications & Requirements:
Active Compact State RN License
At least 2 years current/recent Case Management experience
Special Needs Plans (SNP) experience - HIGHLY preferred
Plan of Care experience
Bilingual - English/Spanish (HIGHLY preferred)
Experience with senior population and chronic conditions.
