We are seeking a highly motivated and independent Registered Nurse (RN) to join our team as a Field Assessor. This role involves conducting in-person assessments for PCA (Personal Care Assistant) and MDC (Major Diagnostic Category) services. The position requires travel within Union and Hudson Counties, with mileage reimbursement provided.

Location: North New Jersey (Union and Hudson Counties)
Schedule: Monday to Friday, 8:00 AM-5:00 PM or 9:00 AM-5:00 PM
The training is for the first two weeks
Responsibilities:
Perform patient assessments to determine appropriate services using established guidelines.
Evaluate the necessity, appropriateness, and efficiency of services provided.
Develop and coordinate care plans tailored to patient needs.
Collaborate with patients, families, physicians, and other healthcare professionals to ensure optimal care delivery.
Monitor patient care activities and outcomes for effectiveness and appropriateness.
Advocate for members and facilitate coordination across care settings.
Educate members about additional resources, such as SNP and MLTSS, when appropriate.
Accurately document all assessments and interactions following organizational policies.
Leverage technology and communication tools to interact with multidisciplinary teams.
Contribute to quality improvement initiatives to enhance member outcomes.
Qualifications:
Active RN license in New Jersey.
Valid driver's license and reliable transportation.
Experience in Home Health or Medical-Surgical settings.
Proficient in technology and comfortable with documentation systems.
Self-motivated, organized, and detail-oriented.
Strong ability to work independently.
Additional Information:
This is a remote field role; there is no office attendance requirement, but travel for member assessments is mandatory.
Average of 3-4 member visits per day.
Mileage reimbursement is provided.
This role is open to RNs only.
